Online After-School Program Application

Thank you for your interest in our after-school programs!

COVID-19 Update: Given the progression of the COVID-19 outbreak and the uncertainty surrounding the upcoming school year, LDANR has made the decision to move all fall programming online to ensure the safety of our clients, staff, and volunteers at this time. This Fall, we will be offering an online reading program in place of our Reading Rocks and Let’s R.E.A.D. programs, an online mathematics support program (in place of JUMP Math), an online self-advocacy and coping skills group (in place of B.E.S.T.) and, for the first time ever, online self-advocacy programming for students in Grades 7-8/Grades 11-12 to prepare them for their transition to secondary/post-secondary. While we understand that in-person services are preferred and proven to be most effective, we are working to provide services that can continue to promote growth and learning during these unprecedented times.

Online Fall program applications will open on Monday, August 17th at 6:00 am!

In the meantime, you can prepare yourself for the application open date by obtaining the following supporting documents that will be required as a part of the application process:

  1. Most recent report card or progress report;
  2. Individualized Education Plan (if applicable);
  3. Teacher Feedback Form (if possible - Click here to access the Teacher Feedback Form);
  4. Any other assessments or information you have that would support your application (e.g., Speech/Language Assessments, Psycho-ed evaluation, Classroom assessments, etc.)
